Username for customer 1.5.x

Introduction

This extension will make a field customer username where the customer can choose username and login with username. No need to change here core file to save time and pay the low cost. Usage
1. upload all files as per root.
2. run SQL file in phpmyadmin (please change prefix)
3. enable module from admin>system>setting> option tab
4. now it's ready for use
